This is so sad. Just shows that one can never be careful enough. My prayers are with his family.

That is sad, but to know the details of the failure would help in preventing future accidents. Think of how many times we're under the overhang, partially standing up. A sudden drop of the rig could prove disastrous.

As a 31 year veteran of the fire service I can tell you this, you can never be too careful. So many preventable accidents happen that change lives forever. Very sad.
