The ceiling fan blade broke when I closed my slide out. Who is the manufacturer of the fan and where can I purchase on blade, it is 16" in length

Try the big box stores. I'm pretty sure they have many different size sets.

Same thing happened to me on my Bighorn. Replaced with this:
Bonus, fan works much much better because it’s not so close to ceiling.
