I found it! The light is made by Command Electronics in Michigan. P/N K-1003. Can be ordered in black or white and with or without the switch. $18.95 each and shipped directly to my door. Much cheaper than taking it back to CW for a repair and waiting for 3 weeks for a warranty repair.
