On my new system Lippert recommends the wires comming out of the power head be zip tied to the power head case. If yours are not zip tied, you might want to try that to prevent the wire from flexing.

The way the wires were restrained inside the powerhead, there was no way they could flex if the outer wires moved. The wire outside the case to the connector wasn't a problem. It looked like the wire wasn't stripped enough or inserted all the way into the crimp and worked it's way out. Zip ties would not have prevented it.
