I don't know about that. Took my dually through the Burger King drive thru last week. I was more worried about the roof than the sidewalls. Worked out fine. Got my grilled chicken sandwich without a hitch.

a: very few...and carefully...usually park & walk in!
b: not! go to the big, open lot
c: rain, truck wash, or custom detail shops

About parking - The way I look at it is this - We have tolerance shoved down our throats daily. We honor diversity - until it comes to my dually, long wheel base truck!!! So, I park it up front and just let my back end hang out. My dually tires on on the lines usually and I don't care. Most parking spots aren't big enough for my regular truck either. I must be getting old 'cause I feel crankiness setting in.
